This training has been designed to provide Health Care Assistants with the knowledge and skills to play an active role in multi-morbidity / long-term condition reviews.

This course will provide training for the following:

• Understanding of BP targets for different groups of patients
• Assessing the regular, irregular, and irregularly irregular pulse and what to do if found
• Administering and understanding the g Asthma Control Test questionnaire
• Checking inhaler technique; teaching good technique
• How to perform a peak flow
• How to administer the online Chronic obstructive pulmonary disease assessment test
• Diabetic foot check (assessment of pulses and neuropathy), including red flags and when to refer
• Understanding the implications of microalbuminuria in diabetes or kidney disease
• 15 Health Care Essentials for Diabetes
• Understanding of which blood tests are needed for which chronic diseases and how often each test needs repeating.
• Understanding of QRISK and when and when not to use it
